Okay this is my last guess but I believe I got it this time ^^... It's Ansem Report 8 becausee it states " As a start I built a device that artificially creates Heartless. By recreating the conditions that spawn the Heartless naturally, I should be able to produce them artificially. This device is the culmination of all my research thus far. The machine's test run successfully created a Heartless."... I looked through all of the Ansem Reports on the game and I believe that's it :3

I will answer that question Maleficient is last seen in Radiant Garden in the chasle of Ansam and Kairi's home and that was in the Room in front of the Keyhole hole room and she died in the Dragon form. However at that time that planet was not called Radiant Garden it was called Hollows Bastion in the first game. After defeating her you aquire the use of the Summon Mushu. This was before Sora Released Kairi's heart from his heart and became a heartless. That was also when Roxas was born and when you play the new kingdom hearts game on the psp you will find out what happened to Roxas from that momment on. So as Sora was saving the Known world Roxas was trying to find out who he was. I would have loved for Rikku to have lost his heart to a heartless and a Nobody version of him was created. To me it would have to be the phantom at the clocktower in neverland, and depending on what color his orb turned determined which attack you would hit him with. If its not him, then my other guesses would be that kurt zisa guy in agrabah or sephiroth in the olympus colleseum

The hardest boss for me had to be the Phantom. It was very frustrating trying to make a continuous link of attacks when he changed his orb color. I didn't die fighting him, it just got me annoyed.
